A 30 minute follow up treatment, after initial acupuncture consultation and treatment.
Cancellation Policy
These T&Cs govern the provision of your 30-minute Follow-Up Acupuncture Session (including Clinical Health Assessment and Treatment) provided by Unity Health & Performance (the "Practitioner/Clinic"). By booking and attending, you agree to these T&Cs. 1. The Service & Duration The service is a 30-minute Follow-Up Acupuncture Session, encompassing a confidential Clinical Health Assessment (medical history review, current concerns, treatment goals) and the Acupuncture Treatment itself as determined by the Practitioner. The 30 minutes includes time for assessment, treatment, dressing, and rebooking. 2. Health Disclosure and Consent Accurate Information: You must provide accurate and complete information regarding your current and past health, diagnosed conditions, medications, supplements, and pregnancy status (or potential pregnancy) during the assessment. Medical Responsibility: Acupuncture is a complementary therapy and is not a substitute for medical diagnosis or treatment by a General Practitioner (GP). You are encouraged to seek professional medical advice for urgent or acute health concerns. Informed Consent: By proceeding, you confirm you understand the nature of the treatment and potential side effects (e.g., minor bruising, slight bleeding), and you freely consent to the proposed treatment. You may stop or modify the treatment at any time. 3. Appointments and Cancellation Lateness: If you are late, the session will still end at the scheduled time, and you will be charged the full session fee. If you are more than 15 minutes late, the Practitioner reserves the right to cancel the session. Cancellation Policy: A minimum of 24 hours' notice is required for cancellation or rescheduling. Cancellations with less than 24 hours' notice will incur a 50% cancellation fee. Failure to attend ("No-Show") without notice will incur the full session fee. 4. Payment Payment Due: Payment is due either before online or immediately after the session. Payment Methods: We accept all debit and credit cards, and cash. 5. Practitioner’s Rights and Confidentiality Refusal of Service: The Practitioner may refuse or discontinue treatment if they deem it inappropriate, if the client is impaired by drugs/alcohol, or if the client behaves abusively. In such cases, the full session fee may still apply. Confidentiality: All assessment information and clinical notes are strictly confidential and processed in compliance with the GDPR.
