Maintenance Plan Terms and Conditions


This document outlines the terms and conditions for the Maintenance Plan, hereafter referred to as '“the Plan” provided by Unity Health & Performance. By enrolling in the Plan, you agree to be bound by these terms.


1. Important Information Regarding Plan Suitability

This plan should only be purchased once discussed and agreed upon with your Unity Health & Performance healthcare professional. In the event of an inadvertent purchase, the Plan will be cancelled and a full refund will be issued, less any processing fees incurred.


2. Plan Overview

The Plan is a monthly rolling subscription and includes two 60-minute osteopathy sessions per month, as well as a treatment programme provided via the Unity App.


3. Payments

Payments for the Plan will be made monthly at £170 per month via invoice, BACS, or automatic payment on the Unity Health & Performance website.


4. Cancellation Policy

4.1. 14-Day Cooling-Off Period

You have the right to cancel your purchase of the Plan for any reason within fourteen (14) days of the date of the initial purchase ("the Cooling-Off Period").

  • To cancel during this period, you must provide written notice to Unity Health & Performance at hello@unityhealthperformance.com

  • Any sessions attended during the Cooling-Off Period will be charged at the standard pay-as-you-go rate, and the remaining balance will be refunded to you within 5 business days.


4.2. Cancellation After Cooling-Off Period

After the 14-day Cooling-Off Period has expired you may cancel the Plan by providing 30 days written notice to Unity Health & Performance at hello@unityhealthperformance.com.

  • The Plan will officially terminate 30 days from the date your written notice is received.

  • You will be responsible for all session fees and payments due up to the effective termination date.


5. Session Scheduling and Missed Appointments

  • All sessions must be scheduled in advance with our reception team.

  • Sessions are non-transferable to other individuals.

  • We require at least 24 hours' notice for any rescheduling or cancellation of a pre-booked session. Failure to provide this notice may result in the session being forfeited.


6. Liability

Unity Health & Performance and its practitioners are not liable for any injury or adverse outcome resulting from the Plan, unless due to negligence on our part. You agree to disclose all relevant medical history and information to your practitioner.


7. Governing Law and Jurisdiction

This agreement is governed by and construed in accordance with the laws of England and Wales. The courts of England and Wales shall have exclusive jurisdiction to settle any dispute or claim arising out of or in connection with this agreement.
