6 Month Osteopathy Plan (1PW) Terms and Conditions

This document outlines the terms and conditions for the 6 Month Osteopathy Plan (1PW) hereafter referred to as '“the Plan” provided by Unity Health & Performance. By purchasing the Plan, you agree to be bound by these terms.

1. Plan Overview

The Plan includes one 60-minute osteopathy session per week for a duration of six (6) months, totalling twenty-six (26) sessions.

2. Payments

Payments for the Plan will be made monthly at £234 per month via invoice, BACS, or automatic payment on the Unity Health & Performance website, or upfront at the patients discretion.

3. Cancellation Policy

3.1. 14-Day Cooling-Off Period

You have the right to cancel your enrolment in the Plan for any reason within fourteen (14) days of the date of the initial purchase ("the Cooling-Off Period"). To cancel during this period, you must provide written notice to Unity Health & Performance at hello@unityhealthperformance.com Any sessions attended during the Cooling-Off Period will be charged at the standard pay-as-you-go rate, and the remaining balance will be refunded to you within 5 business days.

3.2. Cancellation After Cooling-Off Period

After the 14-day Cooling-Off Period has expired you may cancel the remainder of the plan by providing 30 days written notice to Unity Health & Performance at hello@unityhealthperformance.com. Your Plan will officially terminate 30 days from the date your written notice is received. You will be responsible for all session fees and payments due up to the effective termination date.

4. Session Scheduling and Missed Appointments

All sessions must be scheduled in advance with our reception team. Sessions are non-transferable to other individuals. We require at least 24 hours' notice for any rescheduling or cancellation of a pre-booked session. Failure to provide this notice may result in the session being forfeited.

5. Expiry

All sessions within the Plan must be used within 7 months period from the date of enrolment. Any unused sessions after this period will be forfeited and are non-refundable.

6. Liability

Unity Health & Performance and its practitioners are not liable for any injury or adverse outcome resulting from the Plan, unless due to negligence on our part. You agree to disclose all relevant medical history and information to your practitioner.

7. Acknowledgment

By proceeding with payment, you acknowledge that you have read, understood, and agree to the terms and conditions of the 6 Month Osteopathy Plan (1PW).
