Cancellation Policy
TERMS & CONDITIONS These terms apply specifically to the 1.5-hour Initial Injury Rehab Consultation conducted by Unity Physiotherapists, Osteopaths, and Sports Therapists. By booking this assessment, you agree to the following: 1. Service Scope: Initial Consultation The initial consultation is designed to establish a clinical baseline. The session includes a detailed medical history review, physical and functional assessments, and the creation of a rehabilitation plan. You acknowledge that assessments may require you to perform specific movements and may require the clinician to palpate (touch) the affected area. Please wear appropriate clothing to allow for movement. 2. Cancellation and Attendance Policy: We operate a strict 24-hour cancellation policy. Non-Refundable: If you cancel within 24 hours of the start time, or fail to attend (no-show), the session is non-refundable and the full fee will be charged. Exceptional Circumstances: In demonstrable exceptional circumstances the fee may be waived at the sole discretion of Unity management. 3. Medical Disclosure and Indemnity: You are strictly obligated to disclose full details of your medical history, medications, previous injuries, and any other relevant health information. Assumption of Risk: You acknowledge that physical assessment and rehabilitation carry inherent risks. You voluntarily assume these risks. Indemnity: You agree to indemnify Unity and its clinicians against any claims, losses, or injuries arising from your failure to disclose relevant information or from the natural progression of your condition, provided the clinician has not been professionally negligent. 4. Treatment Outcomes Our clinicians use evidence-based diagnostic and treatment methods. However, Unity cannot guarantee specific clinical outcomes, immediate pain relief, or fixed recovery timelines. The success of the rehabilitation plan is heavily dependent on your adherence to prescribed exercises. 5. App Members Area Access: You will retain access to the Unity App Members Area resources only for the duration of your active treatment plan. Access will be revoked upon discharge or if you cease treatment with us. 6. Data Protection: All personal and medical data is collected, processed, and stored in strict compliance with the Data Protection Act 2018 and GDPR. Your data is used solely for healthcare and administrative purposes and is not shared with third parties without your consent, unless required by law.
